It may be comforting to know that from the day you enroll as a member of the NBA, you and your immediate family have a special Accidental Death and Dismemberment (AD&D) benefit provided at NO EXTRA COST.

Benefit amounts are shown as follows:

Accidental death benefits are payable to your designated beneficiary. If a beneficiary is not designated, benefits are payable to the person or persons designated under the policyholder's Basic Group Life Insurance Plan. If neither is surviving, benefits will be payable to the first of the following survivors: spouse, children, parents, sisters/ brothers. If none of these is surviving, benefits are payable to your estate. All other benefits under this policy are payable to the insured.
