 Medical Supplement Plan

Members enjoy a Medical Supplement Plan underwritten by American Public Life Insurance Company. The policy is American Public Life's HI-4000(10/03) NBA-TX.

A supplemental healthcare plan with benefits paid directly to the member. The benefits are designed to help cover your deductibles and co-insurance amounts.

Benefits are as follows:

Hospital Expense Benefit: Pays $30.00 daily benefit for hospital confinement as an inpatient due to a covered accident or sickness, up to a maximum of $180 days per confinement.

Intensive Care/Coronary Care Benefit Rider: Pays $1,000 daily benefit for confinement in a Hospital Intensive Care Unit or Hospital Coronary Care Unit due to a covered accident or sickness, up to a maximum of 20 days per confinement.

Outpatient Sickness Rider: Pays $25 per visit for treatment of a covered sickness by a Physician in the Physician's office, Clinic, Urgent Care Facility or Emergency Room subject to a 5 visit maximum per Covered Adult, except for Covered Dependent Children. The maximum number of visits for all Dependent Children combined is 5 visits per calendar year. The maximum number of visits is 10 per calendar year per family.

All benefits are subject to a 12 month Pre-existing Conditions Limitation. These are plan highlights, so please refer to your Certificate for a full disclosure of Benefits, Exclusions and Limitations.
